Their role might includes connecting people with a range of non clinical programmes, services and events in their local community – like walking groups, mental health counselling, debt management advice, employability skills, volunteering, family fitness, healthy cooking programmes, mindfulness sessions, and the ‘green prescription’ of gardening and food growing.

Community Action Derby is a registered charity providing support and guidance for voluntary and community groups in Derby. Derby City North PCN have partnered with them and employed  Social Prescribers to enable delivery of focused support to the Derby City North residents via the PCN Practices. What is a Social Prescriber?

Social Prescribers connect people with non-medical support, such as day centres, charities or community groups, in the community to improve their wellbeing and tackle social isolation.

Social prescribers work in a GP surgery’s, health centres and community organisations.


 

Social Prescribers can also specialise in one area of care and support, for example supporting people with physical disabilities to be more physically active in the community or people with mental health issues to become involved in counselling and support groups.
