Providing our patients with a holistic approach to care is paramount to us in DCN.

Public Health England’s five year plan aims to protect people by helping them to live longer in good mental and physical health. This includes having a stable and fulfilling job, a safe home and a supportive network of friends and family.

DCN PCN has entered into a Contract with Derby County Community Trust to provide Health & Wellbeing Coaches

DCCT provides a wealth of support for our community with coaching projects, fund raisings events, health education and much more.

So far, two Health & Wellbeing Coaches have been employed and provide services from Derby County Community Trust offices on Pride Park.

Health and Wellbeing Coaches (HWBCs) work alongside patients to coach and motivate them. They support them to become active participants in their own care by self-identifying their needs, setting goals, and implementing their personalised health and care plan.

HWBCs may also provide access to self-management, education, peer support and social prescribing.
